Walk into the pillow aisle of any homeware shop and the choice is presented as primarily about feel: soft, medium, firm. The descriptive language emphasises the cushioning experience, the squish factor, what the pillow feels like when you press it. This is fine as far as it goes, but it almost entirely ignores the variable that determines whether the pillow will actually support your neck through the night. Pillow height is the variable that matters, and choosing pillows on softness alone is part of why so many people wake up with neck stiffness without knowing why.

Why The Industry Doesn’t Emphasise This
The pillow industry sells primarily on feel because feel is what buyers test in stores. You squeeze a pillow, you decide if you like the cushioning, you buy. Height matters more, but height varies based on your specific body geometry and sleeping position, which makes it harder to market in standardised terms.
Manufacturers have responded with vague descriptions: “thin,” “medium,” “thick.” These are roughly useful but lack the precision that would actually help buyers. A pillow described as “medium” might be 10cm thick or 14cm thick, both within the medium category but producing very different effects for side sleepers with different shoulder widths.

The Mattress Interaction
Pillow height and mattress firmness interact. A firm mattress doesn’t let your shoulder sink, so the gap between your head and the mattress surface remains close to your full shoulder width. This requires a taller pillow. A soft mattress lets your shoulder sink into the surface, reducing the gap, so a shorter pillow becomes appropriate.

Quality pillows engineered for better neck alignment often combine materials thoughtfully, with a supportive core surrounded by a softer outer layer, or with adjustable fills that let you fine-tune the height to your specific geometry. These engineered approaches address the height-versus-feel trade-off more directly than single-material pillows do.
The Softness Question Reconsidered
This doesn’t mean softness doesn’t matter. The right pillow combines correct height with appropriate feel. A too-firm pillow at the right height can be uncomfortable on the face and ear. A too-soft pillow at the right height can feel like there’s no support even when the geometry is correct.
The hierarchy should be: get the height right first, then optimise the feel within pillows of correct height. Choosing pillows on softness alone, without considering height, produces matches that feel pleasant briefly but produce stiffness across nights.
For most sleepers, medium-firm pillows in correct height work well, providing enough support to maintain shape while feeling comfortable against the face. Very soft pillows tend to compress under head weight, reducing their effective height below what looked right when uncompressed. Very firm pillows can feel unpleasant against the ear and face, even when the height geometry is correct.
What Goes Wrong With Pillows
The most common pillow problems trace to height issues that buyers attribute to other causes.
Waking with neck stiffness or pain often indicates a pillow that’s wrong for your sleeping position, with the height not matching your body geometry. Switching to a pillow of more appropriate height usually resolves this within a few nights, even with no other changes.
Waking with shoulder pain on the same side you sleep on, can indicate a pillow that’s too low for side sleeping, putting unnecessary weight on the shoulder as it tries to compensate for the inadequate head support.
Frequent position changes throughout the night, often a result of pillow discomfort that the sleeper unconsciously tries to escape. Better pillow height reduces this restlessness because the sleeper can stay in their preferred position comfortably.
Headaches, particularly tension headaches at the base of the skull, can sometimes trace to chronic pillow-induced neck malalignment. The cervical spine isn’t designed to spend eight hours in a slightly bent position, and the muscle strain over years can manifest as headache patterns.
The Replacement Schedule
Pillows have shorter useful lives than most people realise. Most pillow fills compress meaningfully within 1-3 years of regular use, losing the height stability that supports the neck properly. A pillow that felt perfect when new can be inadequate two years later despite looking the same from the outside.
The honest replacement schedule is every 2-3 years for memory foam and latex pillows, every 1-2 years for polyester fibre, and every 3-5 years for high-quality down with regular fluffing and maintenance. Replacing pillows before they fully fail is cheaper for sleep quality than discovering they’ve failed after months of degraded sleep.

For most buyers, the right pillow approach involves: identifying your dominant sleeping position, measuring or estimating the appropriate compressed loft for that position with your specific body geometry, choosing pillows that hold their height stably across their useful life, and replacing them on schedule rather than waiting for obvious failure.
This is more disciplined than how most people choose pillows, and it produces better sleep results. The pillow is a smaller purchase than the mattress, but it’s not a less important one. The mattress and pillow have to work together to maintain spinal alignment through the night, and a wrong pillow can undermine the support a quality mattress is providing.
